B) Targeting the fat loss:
• Reducing the calorie intake: mild to moderate to severe depending on patient’s goal. The caloric intake can vary from 800-
2000 per day
• Fat intake reduction: Most of us eat plenty of fat—more than the body needs. First thing we encourage is reduction of fat
intake in the range of 20-25% of total allowed calories, as per the patient’s goal. High fat content in the body leads to high
insulin resistance, which leads to more fat accumulation, slowing of metabolism and dysmetabolic syndrome. It becomes difficult
to lose weight, leading to further accumulation of visceral fat which leads to gallbladder stones and fatty liver.
3. Carb intake and carb craving:
The most important part of our weight loss program is to cut carbs and reduce carb craving. If our foods provide more than 50%
of calories from carbs, it can lead to the development of abnormal physiological states in body like hyperinsulinemia and metabolic
syndrome. A landmark scientific study of carb cravers demonstrated that these individuals consume 800 calories on average
more per day than people who do not have this syndrome. Unless you can get this problem under control you will not be able
to lose significant amounts of fat. EATING FAT DOES NOT MAKE YOU FAT—EATING EXCESSIVE CARBS MAKES YOU FAT.

4. Mobilizatioin and burning of fat:
The lipolysis and its disposition is done by different
interventions. Increasing the BMR by HCG injection
works via hypo-thalamus, and resets the metabolic
pathway. BMR can also be increased by increasing the
LBM (Lean Body Mass). This is done by increasing the
protein intake with therapeutic protein shakes. Adding
simple exercise and activity can help tremendously.
5. BMR enhancement:
Increased burning of fat can be achieved in several
ways: regular walking or exercise of any kind, 30 minutes
three times a week. Also by adding Cornerstone

6. Lipotropics and Complex Injection:
Lipotropics – (simply means fat burning) are a combination
of B-complex and essential and non-essential

Methionine: assists in the breakdown of fats and

B5 – Pantothenic Acid: a member of the B-vitamin family, is an essential nutrient in human nutrition. It is sometimes referred to
as vitamin B5. Pantothenic acid is involved in a number of biological reactions, including the production of energy, the catabolism
of fatty acids and amino acids, the synthesis of fatty acids, phospholipids, sphingolipids, cholesterol and steroid hormones, and
the synthesis of heme and the neurotransmitter acetylcholine. It also appears to be involved in the regulation of gene expression
and in signal transduction. Roger J. Williams, the discoverer of pantothenic acid and a scientist who pioneered the use of nutrients
for the prevention and treatment of disease, thought that pantothenic acid might be helpful in the management of certain medical
disorders, such as rheumatoid arthritis.
Pyroxidine - or B6: Pyroxidine is a vital part of the enzyme system and very important to the metabolism of proteins, fats and
carbohydrates. It is also vital to the normal function of the nervous and immune systems. The essential amino acid tryptophan
also depends on pyroxidine for its utilization. Adequate amounts of riboflavin and niacin are required in the diet for pyroxidine to
be used nutritionally.
L-Carnitine: boosts energy by stimulating the body’s burning of tryglycerides as fuel, and sparing the supply of glycogen stored
in the liver for heavier exertion.
During exercise, the body will burn fat at a rate of 75-80% of maximum exertion, thus less glycogen from carbohydrates is burned.
L-carnitine allows the body to burn more fat, save more glycogen, and ultimately boost stamina and endurance. By providing
more fat to the muscles, carnitine makes accessible an otherwise unavailable energy source.
B-12 is a vitamin which is important for the normal functioning of the brain and nervous system, and for the formation of blood. It

Body composition Analysis:
The Cornerstone Wellness program utilizes a handheld body composition analyzer (BC IV) to assess patient body composition
and estimate BMR. The BC IV is a Class II FDA-cleared bioelectrical impedance medical device that has a documented accuracy
of+0.5% on average when compared against whole body dual x-ray absorptiometry (DEXA). Algorithims for men, women and
children have been developed and validated by the USC Department of
Biokinesiology. To take a measurement, patients simply grip the BC IV for
6 seconds and results are instantly uploaded, via the internet, to the Cornerstone
Wellness Website for review by patients and physicians. The test
results, coupled with patient goals, are used to create customized meal
plans. Periodic re-tests can be performed and uploaded to track patient

Prescription Diet Program:
On this program you will be prescribed an FDA approved appetite suppressant
medication:
1. phentermine:
What is phentermine?
Oral phentermine hydrochloride (HCL) is sold for weight loss. It is in the
sympathomimetic family of appetite suppressants (used for the short
term management of exogenous obesity.) When used in conjunction with diet, exercise, and behavior therapy, phentermine may
help you to lose weight while you are learning new ways to eat and to exercise.
How does phentermine work?
Phentermine works by stimulating the hypothalamus gland and affecting certain neurotransmitters to decrease appetite. The
hypothalamus is the region of the brain that controls the autonomic nervous system, regulating sleep cycles, body temperature,
appetite, etc. Phentermine should NOT be used as a substitute for proper diet or exercise. For maximum effects, it must be used
in conjunction with a reduced-calorie diet and/or exercise. Any modifications in your diet, activity level, and behavior must be
developed and continued long-term in order to continue losing weight and prevent the lost weight from returning.
2. phentermine and Topamax combination:
Phentermine and Topamax combination has been used to help patient lose weight up to 10% of total body weight. Qsymia is a
similar drug combination based on new study showing good result. Doctors on their own has been prescribing them as a combo
for some time for the benefits of patients.
3.Belviq:
How does Belviq work?
The drug works by controlling appetite — specifically by activating brain receptors for serotonin, a neurotransmitter that triggers feelings of satiety and satisfaction. Serotonin is also involved in mood; many antidepressant drugs work by preventing the
reuptake of serotonin and keeping brain receptors bathed in the chemical. Researchers at Arena say their drug is designed to
seek out only the serotonin receptors that affect appetite.
How effective is it?
According to clinical trial data submitted to the FDA, nearly half of dieters without Type 2 diabetes who used the medication
lost at least 5% of their starting weight — or an average of 12 lbs. — over a year, compared with 23% of those taking a placebo.
For best results, dieters are advised to use the medication together with a healthy diet and exercise program. The approved
labeling for Belviq also recommends that people discontinue the drug if they fail to lose 5% of their body weight after 12 weeks;
longer treatment is unlikely to lead to meaningful weight loss.
Who can take Belviq?
The FDA approved the medication for people who are obese (with a body mass index, or BMI, of 30 or higher) or who are
overweight (with a BMI of 27 or above) and also have at least one weight-related health condition such as Type 2 diabetes, high
blood pressure or high cholesterol.
Are there any side effects?
The most common side effects of Belviq in non-diabetic patients are headache, dizziness, fatigue, nausea, dry mouth and constipation; in patients with diabetes, side effects include low blood sugar, headache, back pain, cough and fatigue.
3. Xenical:
This medicine blocks fat absorption in gut and thus allows fat to be excreted with stool. It is also sold over the counter as Alli.
4.Qsymia
Patients in trials who took Qsymia and made lifestyle changes lost an average of about 10% of their body weight after one year
on the drug, compared to a 2% weight loss in people who took placebo pills. Qsymia, developed by California-based Vivus Inc.,
combines two already prescribed medications -- the stimulant phentermine and the anti-seizure drug Topamax (topiramate) -- in
a controlled-release pill that is taken once a day. Although this loss is considered modest, it was still about twice as much as was
reported in trials of Belviq and some other weight loss drugs.
Side effects included constipation, dry mouth, and tingling in the fingers and toes, but there was no evidence of an increase in
heart disease or stroke risk associated with the drug’s use. People with diabetes or those at high risk for developing diabetes
may benefit most from taking the drug. In the company-sponsored studies, these patients showed improvements in blood sugar
control, along with weight loss.
Other medications, or a natural appetite suppressant may be suggested to help curb your appetite and help you feel satisfied
more quickly. Our Medical Director, Dr. Pankaj Karan, will work with you one-on-one to determine appropriate dosage, as well
as help you learn how to eat nutritiously balanced meals to help you burn fat. And as with all of our programs, you will also
receive one-on-one comprehensive lifestyle education as well as exercise and counseling to help you lose the weight, and most
importantly keep it off.

Here are a few strategies that have been shown to help:

Eat a high protein breakfast and lunch (or substitute with an all-natural high protein shake like the Cornerstone Meal Replacement)
and high protein snacks. This will lower your insulin levels and stabilize your blood sugar, which will help immensely
with carb craving. This will also help maintain muscle, which will keep you burning more calories.
Prepare ahead with crunchy low calorie and slightly salty snacks. At home and work, keep cut up raw vegetables such as celery,
carrots, snap peas, peppers, jicama etc in a lidded container of slightly salty water in the fridge. The crunch and the saltiness will
satisfy the need at a fraction of the calories and insulin response.
Keep cold club soda or sparkling water at home and work with limes or lemons that you cut and throw in. The gas will fill you
up so you will feel full and there are 0 calories. Try not to drink diet sodas as the artificial sweeteners have been shown to make
people seek out carbs and sweets. It has been shown that 27% of calories in America comes from beverages, higher in the South
where sweet tea is one of the 4 food groups. If a 180 lb woman drank club soda instead, and changed nothing else she would
lose more than 50 lbs in a year.
When you eat out always tell the server to skip the bread. Yes, it’s free. But I guarantee you will never not empty the basket.
Also, sub vegetables for the potato. The healthier part of a loaded potato is actually the sour cream and bacon.
If you are a tortilla eater, remember a tortilla has as many carbs and calories as a slice of bread. The average Mexican American
eats 12- 15 tortillas a day. Flour or corn, it makes no difference. Remember, corn is a starch not a vegetable. How does a rancher
fatten a cow? By feeding it corn.
When you go grocery shopping, don’t buy chips, crackers, cereals and other flour and starch based foods. If they are not in the
house you will not eat them. Don’t worry about the kids. Remember your kids have half your DNA—if you’re overweight, they
will likely be overweight, especially if they’re eating what you’ve been bringing home to feed your own carb addiction. Things
are hard enough for kids today. Don’t sabotage your their success in life by contributing to their obesity. If you are not willing
to do it for yourself, at least do it for your kids. Be a role model for healthy eating. Besides, they’ll find enough junk food at the
neighbor’s. And the pesky neighbor kids won’t come over because you don’t have anything they want to eat. Voila!
Met Assist
So far we addressed dietary changes to cure carb craving such as eating more protein, eating more frequently and decreasing
intake of white foods (sugar, bread, rice, potatoes, pasta). There is a supplement called Met Assist which I formulated, which will
help not only with blood sugar stabilization but also help you lose excess body fat. In Met Assist there are 4 major and 4 minor
nutraceuticals that work synergistically to normalize and prevent swings in blood sugar, lower hemoglobin A1C in diabetics,
improve mood, directly curb desire for sweets and actually decrease body fat levels when taken regularly. These nutraceuticals
are as follows:
Hydroxycitric Acid or HCA – This naturally occurring substance is derived from a tropical fruit garcinia cambogia, also known
as tamarind. In multiple scientific studies HCA has been shown to:

• decrease absorption of sugar from the gut so more sugar is simply excreted from the body

• increases serotonin, the “feel good” chemical in your brain which lowers appetite directly as well as indirectly by decreasing
stress and depression which are major triggers for excess eating particularly starchy foods
• lower body weight by BMI by 6% in 12 weeks
Chromium Polynicotinate – This is a compound which consists of the metallic element attached to the B vitamin niacin. This
compound is also known as glucose tolerance factor (GTF) as it is the link that attaches the insulin receptors to the cell membrane
as also keeps the insulin receptors active. When insulin receptors become inactive due to chronic overeating of white foods
this is what causes type 2 or adult onset diabetes. In fact, chromium polynicotinate has been shown in many human studies
to lower hemoglobin A1C (HgB A1C) by 0.6 in diabetics. Diabetes is defined in laboratory studies as an elevated HgBA1C of
greater than 6 so basically this can decrease it by 10% which is quite significant.
